# The Guardrails Behind AI Booking in Healthcare

AI booking should not mean handing an assistant the keys to a clinic calendar.

At Vascue this remains a research workflow, not a public production booking service. These are the release guardrails we would require before real-patient use.

The useful role for an agent is narrow: help a patient discover options, verify who they are, present an exact appointment, and act only after approval. ## 1\. No Direct Access to Clinic Databases

An agent should call a controlled booking gateway, not a clinic's database or practice-management system. The gateway can limit actions, protect credentials, and keep a record of what happened. ## 2\. Minimum Necessary Data

Booking needs the administrative details required to hold a slot for a person, and no more. It does not require clinical notes, a complete medical history, or a list of other patients.

That is both a practical security principle and a privacy-by-design principle. Appointment information can itself reveal health information, so the system should collect and reveal only what is required for the appointment, and the assistant should receive only what it needs to present the option to the patient.

## 3\. Verify the Patient

A one-time code sent to the patient's phone helps prevent someone else from creating or changing an appointment in their name. It is an administrative verification control, not a diagnosis, identity document check, or substitute for the clinic's own clinical processes.

## 4\. Propose, Then Confirm

The system should not silently write to a calendar. It should present the clinic, service, practitioner, time, and relevant booking terms; then require the patient to explicitly confirm.

Only a confirmed proposal becomes a booking. If availability changes, the system must say so rather than pretending the appointment exists.

## 5\. Clinics Keep Control

Clinics decide which services, practitioners, and slots are available. They can require approval, deposits, or other conditions, and they can pause agent bookings.

## 6\. The Agent Does Not Practise Medicine

An agent may facilitate an appointment. It must not decide what care a person needs, make a diagnosis, or present a clinic result as medical advice.

## 7\. Approved Clients Only

Publishing a server card does not open the door. The design is deny-by-default: an assistant or agent platform gains access to booking tools only after technical testing, privacy and security review, and explicit approval, and it identifies itself on every call. A client that has not been approved sees no booking tools at all. That is separate from, and in addition to, the patient's own authorisation; the assistant is trusted to relay, never to decide.

## 8\. Rate Limits and Abuse Controls

Even well-behaved clients can loop, and badly behaved ones will probe. Limits on verification attempts, on proposals per verified phone, and on calls per client keep a mistake or an attack from turning into a flooded calendar. Proposals expire on their own, so abandoned conversations do not hold slots hostage.

## 9\. A Record of Every Action

Because every request passes through one gateway, each one can be recorded: which client asked, which clinic, which verified patient, what the clinic system answered. When a clinic questions a booking, the answer is in the log rather than in a guess.

## 10\. Synthetic First, Then Review

None of the above is tested on real patients. The first evaluation uses test patients and a test calendar, with Cliniko as the first intended integration. Live clinic access requires a separate privacy, security, and provider review, and a clinic's decision to participate. Vascue is ISO 27001 certified, and the [security page](/security) describes the controls already in place; this pilot is held to those same gates before anything reaches a live calendar.

## 11\. Privacy Is More Than a Checkbox

Healthcare privacy obligations depend on jurisdiction, contracts, data flows, and operational controls. A secure API is necessary but not sufficient; documented access controls, auditability, retention rules, incident response, processor agreements, and legal review all have to exist for the deployment in question. ## Where This Leaves Clinics

A clinic considering the research does not need to connect anything. Registering interest on the [pilot page](/ai-agents) creates no production connection and requires no credential. The standard is simple: an agent can help a person complete an administrative task, but it cannot silently create a healthcare commitment.
