MCP, Model Context Protocol, is often described as a way for AI assistants to use tools. That is true, but incomplete.

For healthcare booking, an MCP server should not be a back door into a clinic's database. It should be a carefully constrained translation layer between an assistant and a clinic's existing booking system.

Vascue is researching this architecture; the MCP booking service described here is not publicly live today.

## The Tools an Agent Needs

A booking agent does not need access to clinical notes, billing history, or every patient in a clinic's system. It needs a small set of administrative actions:

-   Find participating clinics matching a location or service.
-   Retrieve currently available appointment times.
-   Verify the patient through a one-time code.
-   Propose a specific appointment.
-   Confirm, change, or cancel that appointment after the patient approves.

That is the Vascue research model: a narrow booking rail, not an all-purpose healthcare data connector.

## What Each Tool Returns

The boundary becomes clearer when you look at the answers, not just the names.

-   **Find clinics** returns participating clinics that match a location and service: name, area, services enabled for agent booking. It never returns a clinic that has not opted in.
-   **Availability** returns open slots at one chosen clinic, drawn live from the clinic's own booking system. It is available only once the patient has verified, so the schedule is not a public feed.
-   **Verify** sends a one-time code to the patient's phone and checks the reply. It establishes that the person in the conversation controls that number, nothing more.
-   **Propose** reserves a specific slot as a pending proposal for a short window. A proposal that is never confirmed expires and leaves no trace in the clinic calendar.
-   **Confirm, change, cancel** are the only tools that write to the practice-management system, and each acts only on an appointment tied to the verified patient.

Five kinds of action, each with a defined input, a defined output, and a defined permission boundary.

## Why Two Steps Instead of One

A single "book this" tool would be simpler to describe and worse to operate. Assistants summarise, retry, and occasionally misread. Splitting the action into a proposal the patient sees and a confirmation the patient gives means the calendar write happens only once a specific human has said yes to a specific slot. It also gives the gateway a natural place to recheck availability, so a slot taken in the last thirty seconds produces a clear "no longer available" rather than a double booking.

## Why MCP Helps

An MCP server gives an agent structured tools instead of asking it to scrape a website or improvise API calls. Each tool has a defined input, a defined output, and a defined permission boundary.

For example, an assistant might ask Vascue for available physiotherapy appointments in Central on Thursday afternoon. The server can return eligible options. It cannot return a clinic's entire patient list, a practitioner's notes, or an appointment that is not actually available.

The assistant then presents the exact option to the patient. The patient chooses. Only then can the booking move forward.

## What the Server Refuses to Do

The list of absent tools is part of the design:

-   No free-form query tool. An assistant cannot ask the server an arbitrary question about a clinic's data.
-   No patient lookup. There is no way to list, search, or browse patients.
-   No access to clinical notes, invoices, or practitioner records.
-   No booking without a confirmed proposal, whatever the assistant claims the patient said.
-   No tools at all for a client that has not been approved and authorised.

## MCP Is Not the Security Layer by Itself

MCP describes how an agent discovers and calls tools. It does not replace identity, consent, access control, or clinical safety.

The surrounding controls matter more:

-   OAuth identifies and limits the client.
-   A one-time code verifies the patient.
-   A proposal-and-confirm step prevents silent calendar writes.
-   Clinic-controlled settings decide which services and slots are eligible.
-   The gateway, rather than the agent, holds clinic-system credentials.

The outcome is intentionally modest: an AI can help a person complete an administrative task. It cannot make a clinical decision, access a medical record, or act without the person's approval.

## Where This Stands

The booking server described above is a research design being tested with synthetic data and test calendars.
