It is tempting to imagine an AI assistant booking an appointment by connecting directly to a clinic's database or practice-management system.

That would be fast. It would also be the wrong architecture.

Vascue's agent-booking work remains a research project, not a public production API. The gateway below describes the intended safety boundary.

## The Database Is Not the Product Interface

Databases contain far more information than an agent needs: patient identities, clinical notes, internal schedules, financial records, staff data, and configuration details. Giving an external agent direct access turns a simple booking request into a broad security and privacy risk.

A patient asking for a Thursday appointment should not grant an assistant the ability to browse an entire clinic's records.

## What "Direct Access" Usually Looks Like

Few teams would literally hand an agent a database password. Direct access tends to arrive in more respectable clothing:

-   A practice-management API key with every scope enabled, pasted into an agent's configuration because narrowing it was fiddly.
-   A general-purpose "query the system" tool that lets the model compose its own requests.
-   Browser automation that logs in as a staff member and clicks through the admin interface.

Each of these gives a probabilistic system the same reach as a trusted employee. Whether the model behaves well on a given day is then the only control left, and that is not a control.

## The Gateway Pattern

The Vascue design places a booking gateway between the agent and the clinic system.

The agent asks the gateway for a defined action. The gateway applies rules before it asks the practice-management system to do anything:

-   Is this clinic participating?
-   Is this service enabled for online bookings?
-   Is the slot live and still available?
-   Has the patient verified their phone?
-   Has the patient approved the exact appointment?

The gateway can return a useful answer without exposing the underlying system or its credentials.

## What the Gateway Adds Besides Rules

Rules are the visible part. Three less visible properties matter as much.

**Credentials stay put.** The clinic's connection to its practice-management system is held by the gateway, never passed to the assistant or the model provider. Rotating or revoking it is a clinic-side action that does not depend on any agent cooperating.

**Every action is recorded.** Because all traffic passes through one narrow door, the gateway can keep an account of what was asked, for which clinic, by which verified patient, and what the clinic system answered. When reception asks "where did this booking come from?", there is an answer.

**Writes are safe to repeat.** Assistants retry. Networks drop. A confirm that arrives twice must create one appointment, not two, and a confirm that fails halfway must be reconciled against what the clinic system actually holds. That logic belongs in the gateway, where it can be tested, rather than in the prompt of every assistant that might ever call it.

## Why This Matters More With Language Models

Traditional integrations call a fixed set of endpoints with fixed inputs. A language-model agent decides at run time which tool to call and with what arguments, and its decision can be influenced by content it reads along the way, including content written by someone hostile. The security term is prompt injection; the practical consequence is that the caller cannot be fully trusted even when the developer is.

A gateway does not stop a model from being manipulated. It caps what manipulation can achieve. The worst outcome of a confused agent calling a five-tool booking gateway is a proposal the patient declines. The worst outcome of a confused agent with database access is a different conversation entirely. ## Least Privilege, in Practice

When a clinic connects its system, the ideal credential is restricted to scheduling. It should be able to read bookable availability and create or manage appointments, but not see clinical notes or financial information.

That same principle applies to the agent. It receives only the data and actions required to fulfil the patient's request.

This is not bureaucracy. It is the difference between a booking integration and a broad, difficult-to-control data integration.

## A Better Question Than “Can the Agent Access It?”

The right question is: *what is the smallest capability the agent needs to help this person?*

For booking, the answer is usually very small. Search. Availability. Verification. Explicit confirmation. Booking.

That is enough to create a useful experience while keeping the clinic's system of record private and in control.
